(1) The actual date the contract is executed by the offeror and the purchaser;
(2) The name and address of the offeror;
(3) The name and address of the trustee;
(4) A complete description of the purchase price;
(5) The expiration date of the contract and the terms and conditions of its extension or renewal, if applicable;
(6) The disclosure required by s. 509.504; and
(7) The disclosures required by s. 509.505.
