As used in this subchapter:

  1. (1) “Healthcare services” means any services included in the furnishing to any individual of medical or hospitalization or services incident to the furnishing of care or hospitalization, as well as the furnishing to any person of any and all other services or goods for the purpose of preventing, alleviating, curing, or healing human illness or injury;

  2. (2) “Health plan” means any group, blanket, or individual accident and health insurance policy, contract, or plan issued in this state by an insurance company, hospital medical service corporation, or health maintenance organization, provided that nothing in this subchapter shall apply to accident only, specified disease, hospital indemnity, medicare supplement, long-term care, disability income, or other limited benefit health insurance policies;

  3. (3) “Inherited metabolic disease” means a disease caused by an inherited abnormality of body chemistry;

  4. (4) “Low protein modified food product” means a food product that is specifically formulated to have less than one gram (1 g) of protein per serving and intended to be used under the direction of a physician for the dietary treatment of an inherited metabolic disease;

  5. (5) “Medical food” means a food that is intended for the dietary treatment of a disease or condition for which nutritional requirements are established by recognized scientific principles and formulated to be consumed or administered enterally under the direction of a physician; and

  6. (6) “Provider” means any person who is licensed in this state to furnish healthcare services as a health professional.
