  1. (a) Any optometrist practicing in any county in this state shall display his or her license in a conspicuous place in his or her office.

  2. (b) Any optometrist practicing away from his or her office shall deliver to each person obtaining services from the optometrist a statement dated and signed by the optometrist setting forth the amount charged, the optometrist's address, and the number of the optometrist's certificate.
