A license or credential when issued shall be in the form prescribed by the Director of the Division of Arkansas State Police and shall include the:

  1. (1) Name of the licensee or credential holder;

  2. (2) Name under which the licensee or credential holder is to operate; and

  3. (3) License or credential number and date of expiration.
