(a) For the purpose of registering the qualified electors of the election districts, the Supervisor of Elections shall prepare and furnish to each board of elections registration cards, serially numbered, in duplicate, and containing spaces for entering the information required by section 100 of this title, and the following affidavit:
I hereby swear (or affirm) that I am a citizen of the United States, that I am not a registered voter in any other state or territory of the United States or in any foreign country or in any other election district, that on the day of the next election I shall be at least 18 years of age and shall have resided for a period of 90 days next preceding said election in the Territory of the Virgin Islands and in this election district for at least 30 days next preceding said election, that I am legally qualified to vote, that I have read (or have had read to me) the foregoing statements made in connection with my registration and that they are true and correct.

Signature of VoterSignature of Board Member or Clerk(b) Subject to the foregoing provisions of this section, the Supervisor of Elections shall prescribe the form of such registration cards.
(c) Each card shall be printed on stock of good quality, shall be of uniform size, and shall be filled out in duplicate for each applicant for registration. The Supervisor of Elections shall provide and furnish to the board suitable methods for filing and indexing the registration cards. His information may be filed, indexed and stored electronically.
(d) The Supervisor of Elections shall develop and implement a uniform and nondiscriminatory primary, official, centralized, interactive computerized territorial voter registration list defined, maintained and administered at the territorial level. The list must include the following:
(1) The computer list must serve as the single system for storing and managing the official list of registered voters throughout the territory;
(2) The computerized list must contain the name and registration information of every registered voter in the territory;
(3) Under the computerized list, a unique identifier is assigned to each legally registered voter in the territory;
(4) The computerized list must be coordinated with other databases in the territory, including the Department of Health and Federal Social Security records; and
(5) The computerized list must serve as the official voter registration list for the conduct of all federal and local elections.
