(a) A hospital shall notify the patient’s caregiver of the patient’s discharge or transfer to another hospital or facility as soon as practicable, but not later than 24 hours before the patient’s actual discharge or transfer to the other facility.
(b) A notification of a patient’s transfer must include:
(1) The name, address, phone number and internet web address of the facility to which the patient is being transferred; and
(2) The name, phone number and email address of the new attending physician working at the facility where the patient is being transferred.
