(1) comply with United States Centers for Disease Control and Prevention recommendations regarding good laboratory practice for CLIA - waived facilities and applicable Occupational Safety and Health Administration regulations; and
(2) provide a copy of its current CLIA certificate issued by the CMS to the Department of Health and Board of Pharmacy.
