Upon receiving a written request from the parent or guardian of a high school student, or a high school student who is eighteen (18) years of age or older, who is attempting to enroll in a postsecondary institution for the purpose of taking one (1) or more postsecondary courses while the student is still enrolled in high school, the high school shall forward to the postsecondary institution to which the student is seeking enrollment a copy of the complete immunization record for such student that is on file with the high school.
The state board of education is authorized to promulgate rules and regulations to effectuate the purposes of this section. All such rules and regulations shall be promulgated in accordance with the Uniform Administrative Procedures Act, compiled in title 4, chapter 5.
