The city or town sergeant or certified constable, upon receipt of the notification as provided in § 9-10-1, shall forthwith make service of the notification upon the persons named therein as jurors by delivering to each of them, or by leaving at their last and usual place of abode, a notice substantially in the following form:

STATE OF RHODE ISLAND

Sc.

TO ......................... Greeting:

You are hereby notified that you have been drawn as a juror for the superior or family court for the county (or counties) of ......................... and you are required to attend the said court be holden at ......................... on the ......................... day of ........................., at ......................... o'clock in the forenoon.

Sergeant.

Constable.

The jury commissioner or his or her agents, having retained the notifications in accordance with § 9-10-1, shall forthwith serve the notifications in the same form as contained in this section by regular mail.
