Expiration date of certificate; renewal; fees

  1. A certificate expires on the second anniversary of its effective date unless the certificate is renewed for a two-year term as provided in this section.
  2. Before the certificate expires, a certificate may be renewed for an additional two-year term if the applicant:
    1. Otherwise is entitled to the certificate;
    2. Pays the department the renewal fee set by the department through regulation; and
    3. Submits to the department a renewal application on the form that the department requires and satisfactory evidence of compliance with any requirement of this chapter for certificate renewal.
