(a) An application for registration as a provider shall be in a form prescribed by the administrator.
(b) An application for registration as a provider shall be accompanied by:
The fee established by the administrator. The administrator shall transmit the fee tothe state treasurer, who shall deposit it in the uniform consumer credit code cash fund, created in section 5-6-204 (1).
The bond required by section 5-19-213;
Identification of all trust accounts required by section 5-19-222 and an irrevocableconsent authorizing the administrator to review and examine the trust accounts;
Proof of compliance with the requirements of title 7 that specify the prerequisites foran entity to do business in this state; and
If the applicant is organized as a not-for-profit entity or is exempt from taxation,evidence of not-for-profit and tax-exempt status applicable to the applicant under the federal "Internal Revenue Code of 1986", 26 U.S.C. sec. 501, as amended.
