(1) It is unlawful for any person:

  1. To make a material misstatement or provide false information to the department during an official disease investigation;

  2. To violate a provision of this part 7 or a rule promulgated pursuant to this part 7;

  3. To aid or abet another in a violation of this part 7 or a rule promulgated pursuant tothis part 7;

  4. To refuse to permit entry or inspection in accordance with section 25-4-710.
