(1) In adopting rules specifying the reporting requirements for entities that receive grants pursuant to this part 8, the state board shall ensure that such reports, at a minimum, include:

  1. An evaluation of the implementation of the program, including but not limited to thenumber of persons served and the services provided;

  2. The results achieved by the program, specifying the goals of the program and thecriteria used in measuring attainment of the goals;

  3. An explanation of how the results achieved by the program contribute to the achievement of the program goals as stated in section 25-3.5-802.

  1. The division shall compile the annual reports received from entities pursuant to thissection.

  2. (a) The division shall annually review the reports received from entities receiving grants pursuant to this part 8 and shall make recommendations to the state board concerning whether the amount received by an entity should be continued, reduced, or increased. The division may also recommend that the grant for an entity be immediately terminated or not renewed if the tobacco education, prevention, and cessation program funded by the grant does not demonstrate a sufficient level of success, as determined by the division.

(b) The division may contract with one or more public or private entities to review and compile the reports received pursuant to this section and prepare the recommendations pursuant to paragraph (a) of this subsection (3).
