(1) Every person in charge of an institution shall keep a record of personal particulars and dates concerning each person admitted or confined to such institution. This record shall include such information as required by the standard certificate of birth, death, and fetal death forms issued under the provisions of this article. The record shall be made at the time of admission. The name and address of the person providing the information shall appear on the record.

  1. When a dead human body is released or disposed of by an institution, the person incharge of the institution shall record the name of the deceased, date of death, name and address of the person to whom the body is released, and date of removal from the institution, or, if finally disposed of by the institution, the date, place, and manner of disposition shall be recorded.

  2. Any person having knowledge of the facts shall furnish such information as he maypossess regarding any birth, death, fetal death, adoption, marriage, or dissolution of marriage upon demand of the state registrar.
