(1) Each application for a supplier license must include, but not be limited to, the following information:

  1. The name and address of the applicant;

  2. The name and address of the supplier and, if the supplier is a corporation, the nameand address of each officer, director, and shareholder holding an ownership interest of ten percent or more;

  3. A description of the equipment and supplies sold or distributed in connection withgames of chance activities in Colorado;

  4. The name and address of the resident agent of the supplier if the applicant does notreside in Colorado and the location in Colorado where the records of the supplier will be available to the licensing authority;

  5. The names and addresses of the Colorado manufacturers and Colorado agents of thesupplier; and

  6. A statement by the supplier or the chief executive officer of the supplier that suchsupplier is familiar with the provisions of this part 6 as to bingo-raffle suppliers and accepts responsibility for compliance with such provisions.

  1. Each application for a supplier license must include a statement regarding whetherthe applicant; its owners; its officers or directors if a corporation; or its members, managers, partners, or associates if another business entity, has been convicted of any felony, theft by deception, or offense involving gambling as defined in article 10 of title 18. A person that has been convicted of any felony, theft by deception, or gambling-related offense as defined in article 10 of title 18 within the previous ten years is ineligible for a license issued pursuant to this section. A person that has been convicted of any felony, theft by deception, or gamblingrelated offense as defined in article 10 of title 18 within more than the previous ten years shall disclose the information related to the conviction required by the licensing authority.

  2. Any bingo-raffle supplier, upon filing a true, complete, written, verified applicationin the form presented by the licensing authority, together with the fee for the license, is eligible for a supplier license. A supplier license shall be renewed annually, on or before March 31 of each year in which such licensee engages in or anticipates engaging in a licensed activity. A supplier license is nontransferable. The annual fee for each license shall be established in accordance with section 24-21-606 (2).
