(1) Every applicant for license by examination shall submit written evidence, verified by oath, and satisfactory to the board that the applicant:

  1. Has not committed an act that would be grounds for disciplinary action against alicensee under this article 295;

  2. Has completed a four-year high school course or the equivalent thereof; and

  3. Has completed and received a diploma from the required approved psychiatric technician education program.
