(1) The board may issue a provisional license to practice as a physical therapist to a person who:

  1. Submits an application and pays a fee as determined by the director; and

  2. Successfully completes a physical therapy program that meets the educational requirements in section 12-285-110 (1)(a).

  1. A person who holds a provisional license may only practice under the supervision ofa physical therapist actively licensed in this state.

  2. A provisional license issued pursuant to this section expires no later than one hundred twenty days after the date it was issued. A provisional license may only be issued one time and is not subject to section 12-285-114.
