(1) The applicant shall take and submit test scores from the board-approved exam. The examination shall be of such a character as to test the qualifications of the applicant to practice optometry.

  1. Each person who makes a passing grade on the practical and clinical examinationand who is otherwise qualified shall be granted a license signed by the board. The license provided for in this section shall be in such form and wording as may be adopted by the board. The optometrist shall display the license for viewing by patients, as provided in section 12-275117. An application for initial licensure as an optometrist shall be accompanied by a processing fee in an amount to be determined by the board pursuant to section 12-20-105.

  2. Any person denied a license under this article 275 and believing himself or herselfaggrieved thereby may pursue the remedy for review as provided under article 4 of title 24 if the action is instituted within a period of sixty days after the date of denial.

  3. A person who fails to pass the examination provided for in this section may retakethe examination the next time the examination is given.
