(1) To renew a license issued pursuant to this article 205, a licensee must submit an application in the form and manner designated by, and must pay a renewal fee in an amount determined by, the director.
Licenses issued pursuant to this article 205 are subject to the renewal, expiration,reinstatement, and delinquency fee provisions specified in section 12-20-202 (1) and (2). A person whose license has expired is subject to the penalties provided in this article 205 or section 12-20-202 (1).
The licensee shall submit additional information that the director requests, includingevidence that the licensee has maintained and holds a current, valid certification from the national certifying agency, to fully and fairly evaluate the applicant's qualifications for license renewal and to protect public health and safety.
