(1) No person, authorized agency, or insurer shall:
Intentionally or knowingly refuse to release any information requested pursuant tosection 10-4-1003 (2);
Intentionally or knowingly fail to provide authorized agencies with relevant information pursuant to section 10-4-1003 (1); or
Fail to hold in confidence information required to be held in confidence pursuant tosection 10-4-1004 (1).
